Layers of Life, Layers of Love

Invitation to participate: Calling ladies with a flare for life! Do you have an unusual job? Have you overcome great adversity in your life? Are you a mother of a daughter? Do you share your home or occupation with more than one generation?

"Layers of Life, Layers of Love" is a women's generational book that will share your story in picture and in prose. I am looking for at least three generations of women in the same family who are willing to share their story. Colorado area is preferred, but willing to discuss other locations. In exchange for your participation in this book project, you will each receive a free 8X12 photograph of your choosing from my website as well as a copy of the published book. If you are interested in participating, here are some forms that you will need.

Women's questionnaire.pdf - This questionnaire will give me information on generations of women.

Minor Child Questionnaire.pdf - This questionnaire should be answered by the mother of the youngest minor child in the portrait series.

Permission To Use Pictures.pdf - This form releases me to use your pictures in the book as well as any written material.
